What We Do
Our Services
We offer intensive, comprehensive residential treatment to young people whose needs cannot be met in community settings. We provide young people with structured, relationship-focused treatment through a residential therapeutic model.
Residential Services:
- All residents reside on our five-acre campus.
- Each space is supervised by a team of highly trained staff with 24/7 supervision to ensure residents’ safety and well-being.
- Our team provides mentorship, symptom monitoring and management, person-centered goal setting, self-direction and moral development to residents.
Our Approach
Through our wide range of solutions, including clinical care teams, versatile intake and referral practices and a better system of communication and transparency between treatment teams and community stakeholders, we are increasing the efficiency and efficacy of treatment.
The ASCEND Program
ASCEND is the first program of its kind in West Viriginia. It was developed locally and is designed to support adolescents through the most intensive emotional, behavioral and mental health challenges.
Because many of the young people we serve are unsuccessful in other residential settings and at high risk for out-of-state placement, ASCEND provides an in-state solution that delivers the additional intensive services our residents need while helping bring them back to their families and communities.
The ASCEND acronym represents five key pillars:
A - Attuned
Our programming provides individualized, evidence-based care attuned to the complex needs of high-acuity young people and their families, with programming designed to respond intensively across all areas of life.
S - Specialized
Our model delivers specialized supervision, clinical oversight and permanency-focused support, integrating family engagement and family-finding to strengthen long-term relational health and resiliency.
C - Clinical Competency
Led by experienced licensed clinicians, our treatment teams provide SAMHSA’s trauma-informed, evidence-based care with strong clinical oversight and ongoing consultation.
E - Experiential Education
Grounded in strengths-based principles, we engage adolescents in purposeful, real-world experiences that build skills, confidence and resilience.
N - Neuro-Sequential Model of Therapeutics (NMT)
Using a neuroscience-informed assessment process, we tailor each person’s treatment plan to address the developmental impacts of trauma and promote long-term regulation and growth.
D - Diversity
The ASCEND program fosters a culture of respect, inclusion and belonging where every young person is valued and supported with dignity.
The ASCEND program structure also incorporates the following program elements:
Staffing Configuration, Support and Supervision
We operate with a high staff-to-youth ratio, including dedicated clinical leadership and family resource roles. This ensures individualized supervision and 1:1 intervention are provided whenever needed.
Cottage Safety
Our cottage model provides increased supervision to support safety, structure and individualized care for youth with complex trauma histories.
Comprehensive Intake and Clinical Assessment
Through a centralized intake process with 24-48 hour responsiveness, young people receive a Neuro-Sequential Model of Therapeutics (NMT) assessment and a full range of standardized clinical evaluations during their tenure at Grant Gardens.
Evidence-Based and Evidence-Supported Models of Care
ASCEND integrates evidence-based and well-supported treatment models of care within our residential care programming.
Substance Use and Trauma Treatment
Using the Seeking Safety model, young people receive present-focused, coping skills based treatment that addresses trauma and substance use through individual and group therapy.
Vulnerable Youth Designation (Family First Prevention Services Act)
ASCEND implements the Not a Number – Love146 curriculum to prevent human trafficking and exploitation while strengthening awareness and resilience.
Family Engagement, Therapy and Education
Family engagement is central to the care we provide. Ongoing therapy, education and Structural Family Therapy principles guide our collaboration to strengthen long-term outcomes.
Family Finding Principles and Permanency Focus
ASCEND incorporates family finding principles through a dedicated Family Resource Specialist to identify kinship connections and build meaningful, lasting adult relationships.
Crisis Support and Intervention
Youth and families have access to 24/7 crisis support, with additional staffing capacity to provide immediate 1:1 intervention during critical moments.
Education Support
Youth attend an on-grounds school with access to individualized educational support, helping them progress academically while addressing emotional and behavioral needs.

The Neurosequential Model of Therapeutics (NMT)
Grant Gardens applies The Neurosequential Model of Therapeutics to all of our residents at intake. This developmentally-informed, biologically-respectful approach is a unique way of working with at-risk children. It is not a specific therapeutic technique or intervention, but rather a way to organize a child’s history and current functioning.
If the resident is in the program for 6 months or longer, a second NMT metric will also be measured in order to gauge the person’s progress.
